November 28 - December 2, 2022. Grades 6,8,10, and 12.

We are conducting a Core Measures Survey of students in grades 6, 8, 10 and 12 surrounding substance use.  This 15 question anonymous survey will help Jefferson County Schools as well as community partners such as Rescue 180 and the Health Department plan prevention and intervention strategies surrounding tobacco and substance use.   The survey will be conducted the week of November 28- December 2, 2022

  • If you do not wish for your child to participate, please fill out the Parent or Guardian Refusal Form attached to the Passive Consent form and return to your child's school by Nov. 22. 
  • No child will be required to take the survey if they refuse.  
  • If parents would like the opportunity to view the questions on the survey they can contact their child's school prior to the survey dates.
